  1. ASSUMPTION OF RISK
    I understand that skateboarding is an inherently dangerous activity that involves risks including, but not limited to: falls, collisions, equipment failure, uneven surfaces, ramps, obstacles, and the potential for serious injury including fractures, head injuries, paralysis, or death.
    I voluntarily choose to participate (or allow my child to participate) in skateboard lessons, classes, and/or camps offered by Northern Lights Skateboard Academy and fully accept all risks associated with these activities.
  2. RELEASE AND WAIVER OF LIABILITY
    I, on behalf of myself (or my child), hereby release, waive, and discharge Northern Lights Skateboard Academy, its owner(s), instructors, assistants, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during participation, including those caused by negligence to the fullest extent permitted by law.
  3. INDEMNIFICATION
    I agree to indemnify and hold harmless Northern Lights Skateboard Academy from any claims, damages, or expenses (including attorney fees) arising from participation in these activities.
  4. MEDICAL CONSENT
    In the event of an injury or emergency, I authorize Northern Lights Skateboard Academy to obtain medical treatment for myself or my child. I understand I am responsible for any medical costs incurred.
  5. SAFETY AGREEMENT
    I agree (or agree for my child) to:
  • Wear a helmet at all times during participation
  • Use appropriate safety gear (pads strongly recommended)
  • Use properly maintained skateboard equipment
  • Follow all instructor directions and safety rules
  • Act responsibly and avoid reckless or unsafe behavior
    Failure to follow safety rules may result in removal from the session or camp without refund.
  1. PHOTO/VIDEO RELEASE (Optional)
    I grant permission to Northern Lights Skateboard Academy to use photos or videos taken during lessons or camps for promotional and marketing purposes (including website and social media).

  2. MINOR PARTICIPANTS
    If signing for a minor, I certify that I am the parent or legal guardian and have the authority to sign this agreement on their behalf and agree to all terms stated herein.
  3. GOVERNING LAW
    This agreement shall be governed by and interpreted in accordance with the laws of the State of Wisconsin.
  4. ACKNOWLEDGMENT
    I have read this waiver carefully and fully understand its terms. I understand that I am giving up substantial legal rights, including the right to sue.
    I acknowledge that I am signing this agreement freely and voluntarily.
